Understanding IRS Form 1095

At the beginning of last year (January/February 2016), you should have received a new IRS form 1095 as a requirement by the Affordable Care Act (ACA).  IRS Form 1095-C for self-insured employers includes information about the health insurance coverage offered to you and, if applicable, your family and whether or not you enrolled in that coverage.

The deadline for employers to furnish the 2016 form 1095-C was extended to March 2, 2017.  Because the deadline was extended, the IRS will allow you to file your income tax return before you receive the Form 1095-C.   Although you do not need to attach a copy of your Form 1095-C with your income tax filing, you may need to submit information from the form as part of your personal tax filing.

Your 2016 Form 1095-C from ¶¶Òô»ÆƬapp has been mailed to your home and should arrive shortly.  If you do not receive the form, please email HR@Fairfield.edu and a duplicate will be printed for you.
